This project falls within the scope of clinical application and basic research in pediatric traditional Chinese medicine. Pneumonia with wheezing is one of the common pulmonary diseases in children, and the World Health Organization (WHO) has listed pneumonia in children as one of the three major pediatric diseases globally. With the development of medicine, although the incidence and mortality rates of pediatric pneumonia have significantly decreased, it still poses a serious threat to the health of children. Currently, domestic research on this subject mainly focuses on syndrome differentiation and treatment, but there are numerous names for the clinical syndromes of pediatric pneumonia, which lack standardization and do not facilitate clinical guidance. Additionally, there is a lack of an objective comprehensive efficacy evaluation system, leading to significant differences in research results. Therefore, a set of integrated Chinese and Western medical approaches, staged syndrome differentiation, internal and external combined treatment, and full-process intervention programs for treating pediatric pneumonia have been developed. Objective clinical efficacy evaluations and systematic studies clarifying the mechanisms of action of traditional Chinese medicine in treating pediatric pneumonia have also been conducted. These efforts aim to further improve clinical efficacy and shorten the course of the disease. It will have significant theoretical significance and broad social value. 1. Dialectical staging and combined internal and external treatment This project studies the laws of traditional Chinese medicine (TCM) diagnosis and treatment for pediatric pneumonia, formulates a TCM diagnostic and treatment plan for 'Pediatric Pneumonia' in the Shanghai Health Bureau, and clarifies the dialectical staging of pediatric pneumonia in TCM: acute phase (wind-heat closure of the lung, phlegm-heat closure of the lung), recovery phase (phlegm and stasis obstruction, spleen deficiency with phlegm accumulation, qi and yin deficiency), and combines it with external treatment methods. Clinical observations found that some children had recurrent coughing, difficult expectoration of phlegm, dark red tongue color, and for the first time, the 'phlegm and stasis obstruction' type was included in the diagnostic and treatment plan. 2. Multicenter clinical verification The 'Pediatric Pneumonia' TCM diagnostic and treatment plan was subjected to a multicenter clinical study involving 300 children, with a control group established. On the basis of basic treatment, the clinical efficacy of TCM dialectical staging combined internal and external treatment for pediatric pneumonia was evaluated. The overall clinical cure rate, acute phase cure rate, and recovery phase cure rate were 100%, 95%, and 78%, respectively. The improvement in symptoms such as cough, phlegm congestion, secondary throat symptoms, complexion, sweating, appetite, bowel movements, and lung signs was significantly better than that of the control group. The time to onset of fever, cough, and phlegm, as well as the disappearance of lung rales, was shorter than that of the control group, which could significantly shorten the course of the disease and reduce hospital stay. 3. Differential diagnosis and acupoint selection, timing of application for pediatric pneumonia, 120 cases of pediatric pneumonia were treated with external acupoint application. Based on the theories of traditional Chinese medicine (TCM) differential diagnosis and treatment, Zi Wu Liu Zhu, and the unity of heaven and man, the method of differential diagnosis and acupoint selection with timing application was used to enhance the efficacy of the treatment. The time to improvement in main symptoms such as fever, wheezing, cough, expectoration, and the disappearance of lung rales in pediatric pneumonia was shorter than that of the control group, promoting the absorption of inflammation in pneumonia. 4. The correlation between tongue image and TCM syndrome type evolution patterns for the first time in Shanghai, using a tongue diagnosis instrument ( The DS01 Traditional Chinese Medicine Tongue and Face Image Acquisition System observed the tongue images of 137 children with pneumonia, using digital tongue image research to explore the evolution patterns of TCM syndrome types in pediatric pneumonia. It revealed that various tongue image parameters are highly correlated with TCM syndromes, providing a foundation for the accuracy of TCM syndrome differentiation and the objective study of the four diagnostic methods. The application study of Qingfei Tongluo Decoction in the treatment of Mycoplasma pneumoniae pneumonia used the 'Tongluo Method' to treat 82 cases of pediatric Mycoplasma pneumoniae pneumonia. The clinical total effective rate was 100%, and the total effective rate of TCM syndromes was 97.56%. Moreover, it significantly improved blood rheology. Basic research found that Qingfei Tongluo decoction regulates the effector IL-17 of TH17 cells, its upstream initiating factors IL-6, TGF-β1, RORγt, and its downstream activators NF-κB. Its effects are superior to azithromycin, reduces MPP mice's PLT, improves blood rheology, and alleviates blood stasis. Published 20 academic papers, cited 53 times. Held 2 national-level training classes, participated in 20 international and national academic exchanges. The treatment plan for pediatric pneumonia based on TCM staging differentiation with internal and external combined therapy has been widely applied in the Fengxian District Traditional Chinese Medicine Hospital and Jiading District Traditional Chinese Medicine Hospital of Shanghai, saving medical costs and achieving significant social benefits. Trained 10 graduate students and 20 postgraduate students.
