Chronic hepatitis B is a serious chronic infectious disease that poses a significant threat to health. The infection route of this disease is concealed, it is contagious, and there are many misconceptions about the disease. Moreover, the treatment period is long, and due to the lack of effective curative methods, patients suffer from tremendous psychological and economic pressure, leading to a significant decline in their quality of life. Our colleagues and I have further confirmed the significantly reduced quality of life among patients with chronic hepatitis B using internationally recognized scales such as the WHOQOL-BREF. Therefore, conducting in-depth research on the quality of life of patients with chronic hepatitis B is of great significance. Quality of life research relies on scales, and the current scales are from abroad. Although they have good reliability, validity, and responsiveness, their sensitivity and specificity may be biased, carrying a clear cultural imprint of foreign countries, and they are not entirely suitable for the Chinese population. Quality of life is culturally dependent. Scale items must be deeply rooted in the soil of local national cultures to fully and truthfully reflect the quality of life status of the subjects being measured. Currently, scales cannot well reflect Chinese people's thinking habits, cultural characteristics, religious beliefs, nor can they capture the connections between symptoms and other organs and orifices during the process of TCM syndrome differentiation and treatment. They are not sensitive enough to assess changes in the efficacy of traditional Chinese medicine (TCM) and struggle to embody the unique features of TCM. However, quality of life is precisely the best bridge for communication between traditional and Western medicine. Therefore, there is an urgent need for us to accelerate the pace of research on quality of life, adopt standardized procedures to develop a quality of life scale with TCM characteristics that can be applied to the evaluation of TCM efficacy and recognized by Western medicine. Since 2004, our project team has first used existing scales to conduct large-scale, multi-center assessments of the quality of life among patients with chronic hepatitis B. The application of a universal scale and a special liver disease scale combined model to assess the quality of life in 335 patients with chronic hepatitis B indicated that patients with chronic hepatitis B significantly lower than normal individuals across all dimensions, especially those with symptoms of blood stasis obstructing collaterals and liver depression spleen deficiency. At the same time, an emotional assessment was conducted on 133 patients, revealing that 37.6% were anxious and 55.9% were depressed, with the damp-heat obstruction type and liver depression spleen deficiency type being the most common. Therefore, based on this foundation, we combined the cultural characteristics of China with the basic theories of traditional Chinese medicine, extensively referenced internationally recognized scales, and constructed a preliminary four-dimensional theoretical model scale based on the clinical characteristics of Chinese patients with chronic hepatitis B. After extensive consultation with experts in liver diseases, statisticians, and psychologists, an initial scale (78 items) was formed, including 23 items for the liver meridian, 21 items for liver collaterals, 18 items for emotions, and 16 items for physical examination signs. Five statistical processing methods were used (t-test, Reaction analysis, Pearson correlation analysis, multiple stepwise regression analysis, principal component analysis) screening to form a test version of the scale. Subsequently, the scientificity of the test version of the scale is tested, 400 copies are distributed, 390 are collected, with 390 valid ones, a recovery rate of 97.5%, and an effectiveness rate of 100%. The average completion time is between 20-25 minutes, making it operable. The split-half reliability is 0.977, indicating that the content of the scale has homogeneity and good reliability. It also possesses good construct validity and responsiveness. A sensitivity analysis of this scale with CLDQ in 180 patients shows a certain correlation but specificity, effectively evaluating the quality of life of patients with chronic hepatitis B. In summary, the newly established Chinese medicine survival quality scale for chronic hepatitis B can serve as a supplement to existing scales, used to assess the Chinese medicine quality of life of patients with chronic hepatitis B, playing a role that other scales cannot replace. It provides powerful and reliable evidence for the efficacy evaluation of chronic hepatitis B treatment with traditional Chinese medicine, quantifying, objectifying, and standardizing the therapeutic effects of TCM for chronic hepatitis B, playing a key role in improving the clinical efficacy evaluation system for chronic hepatitis B. The project has been promoted in key hospitals such as Shanghai Shuguang Hospital, Putuo District Central Hospital, Shanghai Traditional Chinese Medicine Hospital, and Rizhao Traditional Chinese Medicine Hospital in Shandong Province, achieving significant social and economic benefits. The project has published 12 academic papers, 11 of which have been cited 33 times, including 25 citations from other sources and 8 citations from the project itself. It has participated in international and national academic conferences twice. The research results are disseminated through national professional academic conferences and published papers, generating significant academic influence. The project has trained 4 master's degree students. It has also cultivated a team of young doctors who are hardworking, brave to innovate, and dedicated to their work.
