This project belongs to the clinical application research in the field of cardiovascular diseases within traditional Chinese medicine (TCM) internal science. Project Background: Coronary atherosclerotic heart disease is a common type of ischemic heart disease, and its incidence and mortality rates are increasing year by year in China, making it one of the most threatening diseases to human health. Despite extensive research on its pathogenesis and treatment by modern medicine, Western medicine still has limitations in terms of indications, short-term efficacy, high treatment costs, and relatively more side effects. TCM believes that the pathogenesis of coronary heart disease is characterized by a root deficiency and a superficial excess, with qi deficiency as the root and phlegm and blood stasis intermingling as the surface. Based on TCM theory and past 20 years of clinical experience, the project team proposed the method of invigorating qi and promoting blood circulation, resolving phlegm and reducing turbidity as the basic principle for treating coronary heart disease. From this, they formulated the basic prescription for treating coronary heart disease, known as Guanxin Kang. Preliminary clinical studies have shown that this formula is effective in treating hyperlipidemia of the type characterized by qi deficiency, blood stasis, and phlegm turbidity. Coronary heart disease has significant effects on lipid regulation, anti-ischemic myocardial protection, and angina relief. Innovation points: 1. Through standardized experimental design, observe the organ protective effect of Guanxin Kang on ApoE gene knockout mice with early atherosclerosis at the molecular level, prevent plaque formation, or rupture of existing plaques, reflecting the compatibility between traditional Chinese medicine and modern medical treatment models; 2. Select ApoE gene knockout mice, which are highly representative of human atherosclerosis, as the research subjects to study the preventive and therapeutic effects of the traditional Chinese medicine compound preparation Guanxin Kang on early atherosclerosis; 3. Establish a myocardial ischemia-reperfusion injury rat model, and use a traditional Chinese medicine compound with the effects of tonifying qi, promoting blood circulation, resolving phlegm, and reducing turbidity for intervention, to clarify the possible molecular mechanisms of traditional Chinese medicine treatment for this disease; 4. Based on the understanding of TCM syndrome differentiation, evaluate the traditional Chinese medicine preparation with the effects of tonifying qi and promoting blood circulation, The clinical efficacy and possible mechanisms of action, as well as the safety analysis of a traditional Chinese medicine compound with expectorant and turbidity-reducing effects in the treatment of coronary heart disease and hyperlipidemia. Main Content: Based on previous clinical and modern medical understanding of the disease, the project team studied the mechanism of this method in treating coronary heart disease from the perspective of pathological mechanisms. First, a rat in vivo and ex vivo myocardial ischemia-reperfusion injury model was successfully established. In vivo experiments confirmed that this method could reduce the apoptosis index of myocardial cells in rats with myocardial ischemia-reperfusion injury models, regulate the expression of apoptotic factors and chloride channel-related proteins. Ex vivo experiments showed that this method could further open the KATP channels in rat isolated myocardial cells by regulating KATP channel-related genes and proteins. Based on the traditional Chinese medicine theory of 'preventing disease before it occurs', a deep study from the perspective of Western medical etiology confirmed that this method could regulate the blood lipid levels in hyperlipidemic rats and ApoE-/- atherosclerotic mice. Enhance the expression of LDLR and genes in rat liver and peripheral blood lymphocytes; inhibit the formation of aortic plaques in ApoE-/- atherosclerotic mice, reduce levels of inflammatory markers, regulate gene and protein expression related to cholesterol reverse transport in mice. In vitro experiments have shown that this method can improve the secretion function of damaged human vascular endothelial cells, reduce adhesion between endothelial cells and monocytes, and protect cardiac function. A multicenter, randomized, double-blind, placebo-controlled study involving 160 cases has confirmed that this method can significantly reduce the levels of low-density lipoprotein (LDL), cholesterol, and triglycerides in patients with hyperlipidemia. After treatment for 12 weeks, the LDL-C, TC, and TG reduction rates in the Coronary Health group were 18.81%, 48.91%, and 32.86%, respectively, which was significantly better than the placebo group. Moreover, this method can significantly improve symptoms such as dizziness, a heavy head feeling, obesity, and other related conditions in patients with hyperlipidemia. The main therapeutic indicators of traditional Chinese medicine, such as chest pain and other symptoms, are good in terms of safety. Technical promotion and papers patents: During the research period, 35 academic papers were published, including 5 SCI papers that have been cited 209 times; 2 invention patent applications were made, with 1 granted; 4 doctoral students and 6 master's students were trained. This method has been widely applied to Longhua Hospital Affiliated with Shanghai University of Traditional Chinese Medicine, Shuguang Hospital, Yueyang Integrated Traditional Chinese and Western Medicine Hospital, Shanghai Traditional Chinese Medicine Hospital, Ninth People's Hospital Affiliated with Jiao Tong University, and Shanghai Tenth People's Hospital, and has also been promoted and applied to community hospitals, achieving significant social benefits and certain economic benefits.
