This project falls within the scope of basic and clinical translational research in the field of Traditional Chinese Medicine (TCM) internal medicine. Non-alcoholic fatty liver disease (hereinafter referred to as 'fatty liver') is a chronic progressive liver disease and also an undeniable metabolic issue. In addition to progressing to cirrhosis, fatty liver also increases the risk of diabetes and coronary heart disease. Early intervention in fatty liver is an important pathway to reduce the risk of metabolic syndrome and cardiovascular and cerebrovascular events. The hepatotoxicity of statin lipid-lowering drugs limits their long-term use, and the increased weight and bladder disease risk associated with insulin sensitizers restricts further application. Traditional Chinese medicine has gradually become an important choice for the treatment of fatty liver, but existing treatment measures still do not break through the bottleneck problem of 'improving efficacy'. This project aims to make breakthroughs by innovating on the pathogenesis of fatty liver syndromes, conducting clinical translational research and practice guided by the theory of combining syndrome differentiation and disease identification. Over the past decade, we have achieved several important innovative results: 1. Proposed a new model of pathogenesis for fatty liver syndrome. Through multicenter, large-sample clinical epidemiological research and comprehensive data mining techniques, we proposed a new model of fat liver disease pathogenesis that 'spleen deficiency is the basic pathogenesis of fatty liver, damp-heat or damp-heat combined with stasis is an early common phenotype in clinical manifestations of fatty liver; as the condition progresses, the spleen deficiency phenotype becomes more prominent, and most severe patients exhibit a significant spleen yang deficiency phenotype, which is the 'inflection point' for chronic and complex disease progression.' The use of SOM neural network technology has enabled the visualization of classification of symptoms in fatty liver diseases, providing demonstration technology and methods for the study of disease symptom classification. 2. Established a new personalized treatment plan for fatty liver disease based on combination of syndromes and stages and promoted its application. Based on the innovative pathogenesis of fatty liver, combining disease, syndrome, prescription, and efficacy, a multi-center, randomized, The positive drug control trial confirmed the efficacy and safety of Shanghai's first national-level new traditional Chinese medicine, Dan Ning tablets, in treating fatty liver (hepatic steatosis with damp-heat syndrome), with a total effective rate of 84.85%. This provided key evidence for the secondary development of Dan Ning tablets. A new drug, Lipid-lowering Granules (which has been granted a national clinical research approval document), was developed to treat fatty liver (hepatic steatosis with damp-heat combined with stasis). The placebo-controlled, CT evaluation phase II clinical trial confirmed its efficacy rate of 70.19%, which is superior to the control group's 42.57%. A new use for the classic ancient prescription Ling Gui Zhu Gan decoction in treating fatty liver was discovered. On this basis, a key technology for stage-based individualized diagnosis and treatment combining disease pattern identification with behavioral intervention was established, focusing on clearing heat and promoting diuresis (Dan Ning tablets) -- clearing heat and removing blood stasis (Lipid-lowering Granules) -- warming yang and promoting urination (Ling Gui Zhu Gan decoction). This technology has been promoted and applied in ten medical institutions across five provinces and cities including Shanghai, Jiangsu, Anhui, Guangdong, and Gansu. Achieve significant economic and social benefits. 3. Achieve new findings in the study of the pharmacological substances and mechanisms of action of lipid-lowering granules for the treatment of fatty liver. Guided by the corresponding theory of 'Fang-Zheng-Xiao', it was discovered that lipid-lowering granules improve leptin resistance and insulin resistance, regulate FFA and TG metabolism disorders, and have a new mechanism for the treatment of fatty liver. It was found that the main pharmacological component, berberine, has a new target for improving insulin resistance in the treatment of fatty liver. The extract of bitter tea was found to have an LXR antagonist-like effect, providing new evidence for the application of berberine and bitter tea in the treatment of fatty liver. For the first time, the mechanism of action of Linggui Zhugan decoction in regulating lipid metabolism was discovered. The important role of endoplasmic reticulum stress in the formation of sensitivity to fatty liver injury was found, and traditional Chinese medicine prevention and treatment strategies were proposed. During the project research period, a total of 74 papers were published, including 23 SCI-indexed articles with an overall impact factor of 62 points. Cited positively by others 476 times, with SCI citations of 85; reported at international and domestic academic conferences 12 times; obtained 2 new drug clinical approval documents and 1 patent authorization. Trained 28 graduate students and 2 postdoctoral fellows. The project technology has been included in the Shanghai Health Three-Year Action Plan as a major project for community promotion. This project reflects the characteristics and advantages of traditional Chinese medicine (TCM) combining syndrome differentiation and individualized treatment, improves the efficacy of fatty liver intervention therapy, and is a beneficial practice of translational medicine in TCM's participation in the prevention and treatment of chronic diseases.
